Rejuvenate LLC reserves the right to change this notice. Complaints
If you believe your privacy rights have been violated and/or that Rejuvenate LLC or Rejuvenate LLC has not followed this policy, you may file a complaint with Rejuvenate LLC Office Manager or with the Secretary of the Department of Health and Human Services.

Other uses of protected health information
