As we get older the septum which holds the fat in the socket gets weaker, and the fat herniates out. This creates a bulge and a shadow underneath it. At the same time aging results in deflation and descent of the cheek. This exposes the bone that is normally covered by this cushioning. This creates the hollow appearance. What are the treatment options?In general there are 2 treatment options for patients who have under eye hollowness, surgery or filler. What does filler do?Filler is placed in the hollow area in between the fat that has herniated out and the cheek. This smoothens the transition between the lower lid and the cheek. It can last about 1 year. This can be done in the clinic. What does lower eyelid surgery involve?Surgery involves an incision on the inside of the eyelid and the excess fat is surgically removed. I then take fat from your body abdomen or lateral thighs and inject that into the hollow areas to build it back up. Why not use the fat that is removed from the under eye?Sometimes that can be done, but in general you need alot more fat to build the area up then what you can remove from the under eye.

Who is a good candidate for a chemical peel?The TCA chemical peel is a great peel to resurface the skin in patients who have fine wrinkles in their facial skin and especially on the skin around the mouth. The chemical peel is also a good option for patients with sun spots or pigmented lesions that are a result of sun damage over the years. How do I get a chemical peel in Tampa? The first step is to schedule a consultation with Dr Rayess, who will go over your medical history and your skin history to determine if you are a good candidate for the procedure. Then you will likely need to start Retin A for at least 2-3 weeks prior to the procedure to prep your skin for the peel. This helps cleanse the outer layer of the skin so the peel can get in deeper. You will also need to avoid the sun for 2 weeks prior and 6 weeks after to minimize the risk of pigment issues after the peel. What is the process of getting the chemical peel? The peel takes about 30 minutes to perform in the office. It is better to have someone drive you over that day. We ask that you don’t wear any make up. We will then degrease your face and Dr Rayess will proceed to apply the peel. The peel will feel like a bad sun burn when it is applied. Some patients take a Tylenol to relax them during the process. Once your face is sufficiently peeled we will neutralize the peel before you go home. For the first day you won’t have to do anything. On the second day you will start cleaning your face. What is the recovery process?On the first day you go home and relax as there is nothing for you to do. On the second day you will start cleaning your face and applying the ointment according to the postop instructions. Your skin will start to peel on the 3rd day and this will last for 7 days. During this time it is critical that you do not pick at the peeling skin. You cannot apply makeup while your skin is peeling. So some downtime is critical. You must maintain good sun protection for 6 weeks after the peel to minimize the risk of any pigmentation changes. Is it a painful procedure?The most painful aspect is the application of the peel in the office. It feels like a bad sun burn. Once the peel is neutralized the burning sensation will start to fade away. In most patients the scar is barely noticeable. We undermine the skin widely to remove any tension placed on the skin. Dr Rayess also makes sure to evert the skin so it will heal nicely. You will then have to keep it out of the sun, and you can apply silicone cream to help the scar heal. In most patients they have a barely noticeable scar. In addition Dr Rayess breaks the incision up so that it is a z this makes it harder to perceive to the naked eye as peoples vision are attracted to straight lines. Broken lines are harder to notice. Why are males the ideal candidates for this procedure?This procedure is ideal for males because they have thicker skin in their neck. In addition men have hair follicles that can disguise the scar with their beard. It is also a quicker alternative to the facelift. How is the direct neck lift different from a facelift?The incision for a direct neck lift is different from a facelift . The Incision for a facelift performed by Dr Rayess is placed in the hairline. In men this means that hair bearing skin is moved behind the ear and you will have to shave there. That is not a major inconvenience, but something patients need to know about. The direct neck lift avoids that by placing the incision in the center. Both procedures allow us to tighten the muscles in the middle of the neck. How is the direct neck lift different from a facelift?The recovery is pretty simple. It is not a very painful procedure. The main thing is that if we do a platysmaplasty, which is tightening of the muscles in the middle of the neck you should keep your neck still for 14 days to avoid popping any sutures free. We will recommend that you eat soft foods initially to avoid moving your jaw bone. Otherwise you have to take it easy, find a few good shows to watch and enjoy them!
Aging Face? Causes and Treatment options in TampaThere are 3 main changes to the face as we age. Dr Rayess likes to describe them as the three D’s of the aging face; the first one is Descent or Droopiness, the second is Deflation and the third is Deepening wrinkles. Each change has a different treatment option depending on the patient’s anatomy and how severe the changes are.
Treatment of loss of facial volume For deflation you have 2 main options. The first option is synthetic filler. The product is actually based on a molecule that is naturally present in your body in large amounts and the whole process can be done over a lunch break. The product usually lasts about 1 to 2 years. The second option is using your own fat. This requires a procedure where fat is harvested from areas where you have persistent fat and then injected into your face and has a slightly longer recovery time. Your ideal option depends on your time frame, how quickly you need to get back to work and the level of deflation. Treatment of Jowls and facial sagging For descent or drooping of tissues you really need a lifting procedure and your main options are either a lower facelift or lower neck lift depending on where you’re most bothered by the sagging. Some people are using a thread lift. While they do provide some short term results their longevity is debatable. Dealing with wrinkles Lastly we have deepening wrinkles.
