- Descent or droopiness of tissue. This is sagging of tissue due to loss of elasticity and results in the formation of a loose neck, jowls and deepening of your nasolabial folds or smile lines.
- Deflation this results from loss of volume both fat and bony loss. To counter this you need to add volume back and your options are filler or fat grafting to the face using your own fat. Dr Rayess performs both of these procedures.
- Deepening wrinkles fine lines on the face start to get deeper. These wrinkles are often the result of sun damage to the skin. To treat these you need a resurfacing procedure such as a chemical peel or laser.

Where are the incisions for the facelift and will there be a scar?

The facial ligaments are fibrous attachments that extend from the bone or deep fascia and attach to the dermis which is the underlayer of the skin. These ligaments serve as anchor and support points for the face.

What this involves is releasing the skin and the thick fascial layer that is deeper than the skin also known as the SMAS from their attachments and repositioning them to their natural position. All the tension is placed on the SMAS which is a thicker fibrous layer and will be able to retain it longer. Thread lifts or PDO thread lifts use barbed wire to lift up tissue that has sagged down. These wires are dissolvable and are not very effective when there is significant sagging of tissue. They do not reposition the deeper tissue such as the SMAS which can hold tension well.
